Step 1: Scan the QR Code using your mobile phone and complete the required information.
Step 2: Our HMO Team will process your request and secure the necessary HMO approval before your scheduled service.
Step 3: Once approved, claim your Letter of Authorization (LOA) at the HMO Office (Commercial Building) before proceeding to your diagnostic procedure.

II. CONSULTATION / CHECK-UP
Step 1: Proceed to the Information Desk for doctor assignment (doctor decking).
Step 2: Go to the HMO Office for HMO eligibility verification and approval.
Step 3: Proceed to your assigned clinic.

IV. ADMISSION / CONFINEMENT
Step 1: Proceed directly to the Emergency Room (ER) or Admitting Section.
Step 2: Present your HMO card and a valid ID.
Step 3: The Admitting Section will coordinate with the HMO Office for eligibility verification and approval.
Step 4: Once approved, proceed with hospital admission.
